Title: Residential Clinician - Riverbend Center

Salary $: Part-time, pay rate negotiable depending on experience, work weekends,

some evenings and fill in for vacations and sick leave

General Duties:

  • Interactions with residents reflect competency in counseling and assessment and advance goals set forth in the ISP. Performance indicators:
    • Diagnostic assessment and formulation of ISP.
    • Individual counseling.
    • Group counseling.
    • Spontaneous interactions and activities.
  • Judgements made about residents or activities on resident’s behalf are appropriate both to the individual’s functioning and the agency’s risk tolerance standards.  Performance indicators:

Designates initial site/supervision orders for each resident and makes necessary subsequent changes.

  • Admission, discharge and step-up decisions.
  • Reviews available information prior to admission and during the course of stay.
  • Documentation is complete and timely.  Insures that sufficient psychosocial referral information is available as soon as possible. Performance indicators:
    • Intake paperwork, progress notes, billing logs and discharge paperwork.
    • Facilitates the acquisition of an adequate amount and quality of referral information.
    • Makes collateral contacts to gather more psychosocial information.
  • Documentation is clinically accurate and substantive.  Meets agency productivity standards.  Performance indicators:
    • Diagnostic assessment, mental status exam, progress notes, billing logs, risk assessment and termination summaries.
    • Billing logs refer to ISP.
    • Productivity as defined as total time worked divided by hours of documented billable activity.  Standard for this job 30%
  • Interactions with essential others is constructive and services to advance the quality of care, the therapeutic milieu in the facility and the agency’s standing in the community.  Performance indicators:
    • Peers and supervisors.

Qualifications:

Education/Experience: LSW, LISW, LPC, LPCC Masters degree preferred.  Mental health experience required. Substance abuse treatment experience an asset.

Title: Program Supervisor         

Salary $: Full time, salary negotiable depending on experience         

General Duties:

Provides over all program development, direction and quality service measurements.  Provides clinical and administrative supervision to case managers and case aide, with an emphasis on the  development and enhancement of case management and documentation skills. 

Responsibilites:

  • Maintain a caseload of high acuity clients.
  • Provide at least two hours per week of clinical supervision to each case manager and one hour of supervision per week to case aide.
  • Manage and perform job duties in accordance with the program purpose and within the general policies of the agency.
  • Forge relationships with the persons served and develop strong collaborative partnerships with various social service agencies.
  • Develop, write, manage and administer all Ryan White and HOPWA grants.
  • Initiates, plans and develops new or improved activities to ensure high-quality service to all clients in the program.
  • Ability to work as a team member within the program and the agency staff.

Qualifications:  

Education/Experience:

  • LISW-S in the State of Ohio
  • Must be familiar with ODH requirements.
  • Strong working knowledge of community resources.
  • Culturally sensitive to individual and families served within the program.
  • Demonstrated understanding of community resources and how to access them
  • Excellent relationship building skills to keep participants engaged in services
  • Good written and verbal communication skills
  • Attention to detail, strong organizational, time management and problem solving skills
  • Must have proficiency with computers, including Microsoft Office

Miscellaneous requirements:  Criminal background check, clean driving record; TB Test, CPR/First Aid training, clean drug screen.  Must be willing to transport clients in personal vehicle.  Vehicle must be covered by state minimum insurance.

General Duties:

  • Be aware of the whereabouts of residents and visitors during shift.
  • Note adherence to program rules; maintain order.
  • Log significant changes in resident's mode and/or behavior.
  • Contact staff person on-call regarding unusual resident's needs and/or emergency situations.
  • Transport residents for leisure activities and residents' errands deemed necessary by therapists and/or supervisor.
  • Participate in leisure activities with residents, e.g. picnics, swimming, board games, volleyball, etc.
  • When needed, help residents to stay "on task" with activities including but not limited to, dishwashing, cooking, chores, and laundry.
  • Secure house and offices appropriately during scheduled shift.
  • Answer telephone, taking messages when appropriate.
  • Report any equipment malfunction in the log.
